If your employer offers an Employee Assistance Program (EAP) through Revive, formerly BHS, you may already have free, confidential therapy sessions available to you, and sometimes to your family. The quickest way to check is your benefits portal or HR office. Proactive Wellness participates with Revive (BHS), so once you know your benefit, you can contact us directly or call Revive and ask for Nikita Banks, LCSW.

What is an EAP?
An Employee Assistance Program is a benefit your employer pays for that offers short-term counseling, referrals and other support for work and personal concerns. You usually don’t pay anything for the sessions your employer covers. The number of sessions and who qualifies are set by each employer, so two people with Revive can have different benefits.

BHS is now Revive
BHS was acquired by Revive, and many employers now describe their EAP as “Revive & Thrive, powered by BHS,” “Revive (BHS)” or simply BHS. If your benefits materials mention any of these names, they’re referring to the same network. Older brochures may still say BHS.
Employers that publicly list Revive or BHS
These are examples, not a complete list. Each comes from the employer’s own public materials, and benefits can change, so always confirm your current coverage.

How to find out what your employer offers
- Check your benefits portal or handbook. Search for “EAP,” “Employee Assistance,” “Revive,” “BHS,” “Lyra Health” or “Spring Health.”
- Ask HR or your benefits contact. Ask which EAP you have and whether family or household members are included.
- Call the EAP. Revive’s member line is 800-327-2251. You can ask for Nikita Banks, LCSW, by name.
- Or contact us. We’ll explain what we need to verify your eligibility and any authorization before your first session.
Who may qualify
Depending on your employer, eligibility can include:
- Full-time and part-time employees
- Spouses, partners and dependents
- Household members
Some employers limit eligibility by job classification or location. We check your specific benefit rather than assume.
What therapy can help with
People use EAP therapy for anxiety, depression, burnout, work stress, relationship strain, family stress, grief, life transitions and substance-use concerns. Our EAP work is practical and action-oriented, and we specialize in culturally responsive care for professionals of color, including those who are often the only one on their team.
- Cognitive behavioral therapy (CBT) for anxiety, stress and depression (APA).
- Trauma-focused therapy after difficult or frightening events (VA/DoD 2023).
- CBT for insomnia (CBT-I) for ongoing sleep problems (ACP 2016).
- Motivational interviewing for drinking or other substance use (SAMHSA TIP 35).
Where you need to be
Proactive Wellness provides therapy by secure live video. You need to be physically located in New York, New Jersey, Georgia or Massachusetts during each session.
Keep your information private
When you contact us, share only what’s needed to start: your name, contact information, employer and which EAP you think you have. Please don’t post health details in public comments or social media messages. We’ll collect the rest through our secure intake.
DOT SAP evaluations are separate
If you need a DOT Substance Abuse Professional (SAP) evaluation after a drug or alcohol violation, that’s a separate service from EAP therapy, with its own federal rules, reports and payment. Some employers arrange SAP evaluations through Revive (BHS) and many don’t, so don’t assume your EAP covers it. Learn about our DOT SAP services.
