What happens after a DOT drug or alcohol violation?
Initial evaluation
The SAP completes an individualized clinical assessment and explains the required process.
Recommendation
You receive and complete the required education, treatment or both, based on the evaluation.
Follow-up evaluation
The SAP reviews documentation and determines whether the recommendation was completed successfully.
Return-to-duty test
If an employer chooses to proceed, the employer arranges the required return-to-duty test. Follow-up testing applies after return to safety-sensitive duty.
$500 for all professional SAP services.
The fee includes the initial evaluation, individualized recommendation, required initial report, follow-up evaluation, compliance report and follow-up testing plan. Outside education or treatment, laboratory or return-to-duty testing, and third-party fees are not included.
Several payment options are available, and qualifying employer benefits may cover eligible SAP services.

Verify a SAP before you pay.
A professional license alone does not make someone a DOT-qualified SAP. Ask the provider to confirm each requirement:
- An eligible professional credential under 49 CFR §40.281
- Clinical experience with alcohol- and controlled-substance-related disorders
- DOT-specific qualification training
- Successful completion of the required qualification examination
- At least 12 hours of relevant continuing education every three years
- Current documentation that can be provided to DOT representatives, employers or C/TPAs on request

Can I start if I am not currently employed?
Yes. You may begin as a self-referred client. A current or prospective DOT-regulated employer is needed later to arrange the return-to-duty test and make employment decisions.
Can the complete process be finished within 24 hours?
No. Initial scheduling is often available within 24 hours after intake and payment. The complete process depends on the individualized education or treatment recommendation, completion and follow-up evaluation.
Does the SAP administer my return-to-duty test?
No. After the SAP reports successful compliance, the current or prospective employer arranges the return-to-duty test.
Is virtual service allowed?
Remote SAP evaluations must comply with federal requirements and the geographic limits of the SAP’s professional credentials. Proactive Wellness serves clients located in NY, NJ, GA and MA.
