SAP fees vary widely, and so does what’s included. Before you compare prices, make sure you’re comparing the same services. A low price that leaves out the follow-up evaluation can end up costing more.
What a complete SAP fee should cover
- The initial face-to-face evaluation
- Your written recommendation for education or treatment
- The initial report to your employer (or to you)
- The follow-up evaluation after you finish treatment
- The compliance report and follow-up testing plan
Our fee: $500 for all professional SAP services
Our $500 fee includes every item above. Your first inquiry is free, and payment arrangements may be available. Payment is required before your formal evaluation is scheduled.
Have an employee assistance program through Revive (formerly BHS)? You might be eligible to have your SAP services covered. We’re a contracted Revive provider, so ask us to check before you pay, or call Revive at 800-327-2251 and ask for Nikita Banks, LCSW, SAP by name.
Other costs to plan for
- Education or treatment. Cost depends on what’s recommended and the provider you choose. Health insurance may help with treatment.
- Return-to-duty test. Arranged by your employer. Who pays depends on employer policy.
- Follow-up tests. At least six in the first year. Payment depends on your employer or agreement.
- Clearinghouse and license fees. CDL holders may face state reinstatement fees.
Watch out for these pricing traps
- A low initial fee with an expensive follow-up evaluation charged later
- “Package” pricing that bundles a specific treatment program before you’ve been evaluated
- Promises of fast clearance for an extra fee
A SAP must make an independent recommendation after evaluating you. No legitimate SAP can sell you a pre-set treatment plan.
Can someone else pay?
DOT leaves payment to employers and employees. Some employers or benefit programs cover eligible SAP services. Learn when an EAP can help.
Frequently asked questions
Is the follow-up evaluation extra?
Not with us. Both evaluations and reports are included in the $500.
Do you take insurance for SAP evaluations?
SAP evaluations are usually not covered by health insurance because they are regulatory assessments. Treatment may be covered.
Why pay before scheduling?
It reserves your evaluation time and covers preparation. It’s our policy, not a DOT rule.
