Pilots: SAP Evaluation vs. FAA Medical Clearance

For pilots and other FAA medical certificate holders, the SAP process and FAA medical clearance are separate. Here is how they fit together.

If you’re a pilot or another FAA medical certificate holder, a DOT violation starts two separate tracks: the DOT SAP return-to-duty process and the FAA’s review of your medical certificate.

Track 1: The DOT SAP process

Like every DOT safety-sensitive employee, you complete a SAP evaluation, the recommended education or treatment, a follow-up evaluation, a return-to-duty test and follow-up testing. This makes you eligible to return to safety-sensitive duty under DOT rules.

Track 2: FAA reporting and medical review

For employees holding an FAA medical certificate under 14 CFR Part 67, verified positive drug tests, alcohol violations and refusals are reported to the FAA within two working days. Pilot violations are also entered in the Pilot Records Database.

Completing the SAP process does not by itself restore your medical certificate. You may also need FAA medical clearance before you can use your certificate again. Many pilots work with an Aviation Medical Examiner, and some go through the FAA-recognized HIMS program.

How to keep both tracks moving

  • Start your SAP evaluation promptly
  • Tell your SAP you hold a medical certificate
  • Talk with an Aviation Medical Examiner experienced with FAA substance-related cases
  • Keep copies of your SAP reports, which you can request from your SAP

Other aviation employees

Flight attendants, mechanics, dispatchers and other covered aviation employees follow the SAP process. Refusals by holders of FAA pilot, mechanic and certain other certificates are also reported to the FAA.
